An insurance certificate must be provided to the Town by individuals or companies that work on or permit the use of Town property or Town facilities, or supply goods and services to the Town. Insurance requirements are set out in the agreement, contract, licence, permit or purchase order with the Town.

The Town requires that a Town of Ajax Certificate of Insurance form be completed in the following instances:

  1. Certificate of Insurance – General
    • To provide proof of Commercial General Liability and/or Automobile Insurance and/or Professional Liability only
  2. Certificate of Insurance – Bid Documents/Contract Awards
    • To provide proof of Commercial General Liability and/or Automobile Insurance and/or Professional Liability and/or Environmental Liability only
  3. Certificate of Insurance – Sediment & Erosion Control Permits
    • To provide proof of Commercial General Liability and Environmental Liability only

The following sections of the Town of Ajax Certificate of Insurance must be completed:

Named Insured

  • Full legal name of the insured and mailing address. 

Location and Description of Work/Activity

  • List the Bid Document, name of the project, or the event and the location or facility where the work or activity is taking place.

Coverage

  • The types of insurance required and limits of liability are set out in the agreement, contract, licence, permit, or purchase order with the Town.  Requirements vary based on the risk associated with the work or service or event.
  • The insurance company must be licensed to operate in Ontario.
  • The insurance policy must provide coverage throughout the term of the agreement. Renewal certificates must be provided annually if the agreement extends beyond the expiry date of the initial insurance certificate.

Additional Insured

  • All insurance certificates submitted to the Town must list “The Corporation of the Town of Ajax” as an Additional Insured.

Authorization

  • The Town of Ajax Certificate of Insurance form must be completed and signed by an authorized insurance representative or official at the insurance company or broker.

Other Certificates of Insurance

  • The Town will accept Certificates of Insurance on other forms provided by insurance companies/brokers for all other types of insurance provided that the insurance certificate meets all of the Town’s requirements as set out in the agreement, contract, or permit.
